details will appear (Disease Name, Date, Notes).<\/li>\n\n\n\n<li class=\"translation-block\">To modify the record, click the&nbsp;<strong>Edit&nbsp;<\/strong>icon.<\/li>\n\n\n\n<li class=\"translation-block\">To remove the record, click the&nbsp;<strong>Delete&nbsp;<\/strong>icon.<\/li>\n\n\n\n<li>After editing, save the changes.<\/li>\n<\/ul>\n\n\n\n<figure Update<\/strong>&nbsp;button.<\/li>\n\n\n\n<li class=\"translation-block\">From the&nbsp;<strong>Chronic Diseases<\/strong>&nbsp;dropdown list, select the required disease.<\/li>\n\n\n\n<li class=\"translation-block\">Select the&nbsp;<strong>Date<\/strong>.<\/li>\n\n\n\n<li class=\"translation-block\">Enter the details in the&nbsp;<strong>Update<\/strong>&nbsp;field.<\/li>\n\n\n\n<li class=\"translation-block\">To upload a document, click the&nbsp;<strong>Choose File<\/strong>&nbsp;button under&nbsp;<strong>Attach a File<\/strong>&nbsp;(allowed formats: PDF, DOC, DOCX, PPT, PPTX,
